Work permit for Common law - Urgent

Hi,
My girlfriend and I have been living together for the past year together in Toronto. She is working under NOC 1221 and has a valid work permit till Feb 2012. I too have a valid work permit till March 2013. But the issue is my company closed down last month so I am currently out of a job.
I was thinking of applying for an open work permit using the common-law option but how do I go about it? What documents do I need to show and is it an actual option for my case?
