Work experience

Hello All,
I am new here. got one question to seek answer from seniors.
I have more than 10 years work experience (14 years) in different companies. So if I write reference letter, do I need to provide those from 10 years ago? Say the employer I worked in 1995? or just need recent 10 years'?

what about those companies whom have closed down or non contactables? cause i have moved in a few jobs in a few countries? Is it ok just to write an explaination that these employers cannot be contacted or have become defunct because of the economice downturn?
