does anyone have an idea why they take months after medicals to grant the visa?

If background checks are done before medical, process will be quick and vice versa

Agree with ana_ana and asyousuf.
The Immigration Section of Canadian visa office handles a large number of application files every day. They  follow an office procedure laid down by the Government. Therefore the files only will move according to the maintained queue system. The factors delaying the cases are of course related to individual problems found while review of files. If everything is perfect a final decision will be fast.

The delay after medicals is due to following reasons.

1. Delay in receipt of medical papers by Visa Office
2. Some of Medical papers need extended verification
3. Frequent travelling seen in passport which means more background check needs to be done.
