Pls advise if I am using extra sheet of 5406, do I have to refill all contents or just write what need to be filled in respective columns and rest with N/A.

Just write what you need to write for that particular question only if rest have already been filled.

write your name at top of forum name like 5406 , then dont forget to give reference of question that you are filling.

I think i read some where, also sign extra sheet that you attach, not sure let me look up for this or meanwhile any senior will confiirm.

On additional sheets, i wrote my name, the name of the form and form number.

Under that, I wrote down the question number.  Say I was filling in a table and it had five lines but i need 8. I wrote all 8 in the additional sheet. I don't think its necessary to do that, just the part that is not in the actual form (i.e. 3 lines with the headings).
