The B4a is a continuation of the B4 for list that won't fit on the first page.  Look in the center.  There is a box you check if it is goods to follow.

 The forms are self explanatory. please download the forms from the below given links:

What PMM explained is correct as the B4A is to be used as an additional form if required.
