I have a question about which applications to fill. Do I have to fill the applications like 5604 and 5409 even though that they dont apply for me ( No kids left behind and I am married). And also i would like to know do I need to translate the seal from the notary( its not in English).

You don't need them if not applicable to you.
Translation of every part of the document is necessary.
