Author Topic: Where do I get the Appendix form to bring to Medical exam?  (Read 241 times)

I was told my husband needs to bring the Appendix form to his medical exam.  I looked online and saw a form called Appendix C for people outside of Canada being sponsored to fill out to bring to the medical exam.  I tried to print it but it wouldn't work.  Anyone know where is the correct page where I can print it from?  we are in the US.  Thanks.

Appendix C IS the form he needs.  Look on the Regional Guide for the US for the one for persons in the US.

just to confirm, the link above are the special instructions for spouses living OUTSIDE of canada...

i too was confused after getting wrong information from a dmp, after asking questions here they were answered explaining we must submit the medical with our application

But if you are in Canada as a tourist let's say, and you will apply OUTLAND, when you go to take the medical exam in Canada you bring the form that is for "OUTSIDE application", right?
