What to check/tick on the form IMM1249

Hi just want to ask which box I need to check on the Form IMM1249. A, B, C, D or E. Applying Extend/Change Conditions with new Employer.  I am under LCP and just done 2 years last May and my current employer will release me this June 30, 2011. I found a new Employer and LMO is on-process. I just prepare the forms while waiting for the LMO response. Thanks.

Thanks for the response job_seeker.  I still have Work PErmit with my employer but they will release me this June 30, 2011. My Work PErmit with them is valid until April 30, 2012. Thanks a lot.
