You can find the forms for Manitoba PNP on the Manitoba PNP website at http://www2.immigratemanitoba.com/browse/howtoimmigrate/pnp/application_forms.html

I know they say they include the forms for CIC too in that package but after you get your PNP nomination and are ready to apply for PR, you should look at http://www.cic.gc.ca/english/immigrate/provincial/apply-how.asp for the most recent version.
