What  is the difference between sending RPRF with the medicals and receiving medicals without it  is this denoting something or just a different aspect between the visa offices ?

RPRF need to paid before stamping the visa. If there is no RPRF requested by Visa Office with medical advice, you can expect along with PPR.

Right wilson, some Visa offices ask MR request + RPRF + Passport request too (example New Delhi VO), some VO asked MR request and review medical results than issue RPRF request and PP request like Buffalo VO.
