we did our exam with chmiel in Jan 2011, and he still has not sent it out yet (June 21.2011)!!!! after over 5 months!! we received a letter from the regional office stating that if we do not take action within 60days, we would get rejected. we tried to get hold of him and he is not readily available and always promise to mail it out tomorrow after tomorrow. We contacted the regional office to obtain another set of forms for another exam but no reply yet. what is wrong with this old guy? no sense of trust, responsibility, or professionalism. what shall we do?Huh help!!!!

Chmiel is not even listed as a DMP in Chicago these days ... the 2 Chicago DMPs are:

- Dr. Serafin Veramendi
- Dr. James Arons

probably your medicals were the last that Dr. Chmiel did before being disqualified as DMP ... sorry to hear your story

I suggest you do your medicals again if possible ... you don't need medical forms from Visa Office ... contact the above DMPs and ask them if they have the required forms (if they have the forms they'll be happy to do your medicals) ... though you should have your Immigration File Number ready and other info about your application (better take your previous medical instructions letter to them)

I'm close to Chicago but I didn't do my meds from above DMPs cz I found a DMP in St. Louis, Dr. David Campbell, who was less expensive than the above two ... sent the results to Ottawa through DHL within 20 days as you can see in my sidebar ... you can give him a try if you want

I called one of the other DMPs in chicago and he said we will need the medical forms from the visa office.

also, our family doctor suggested that we can make a complaint through IL Dept of professional regulation

Chmiel is such a shame on the medical profession and we are so frustrated.

how long does it take for the DMPs to get the file ready for mailing? last time we called Chmiel he says he is still working on it and our call bothered his work. Is it a lot of paperwork? I saw an applicant waited in his office for six hours just to get it mailed out. no sure we shall do the same thing coz we do not know whether he has even completed the paper work yet. any ideas? He said he was sick for months. but dear, 5 months delay?Huh!!! have you considered the feelings of applicants? Do you know the word PROFESSIONALISM?Huh? and god, his English~~A real challenge to your ear. and his temper, irritation to your level of tolerance.

Sorry to hear your case......but he did the same thing to me. I did make a complain to CIC earlier. In my case he sent the medical almost after 2 months. You have to go to his clinic and make sure he has completed your file and sent it. No use calling....nobody picks up. In short go to his office......and pester him. Sad to know CIC has not taken action against this bogus man, unscruplous he is.....with no work ethics.

Write a letter to CHC asking them to resend the medical requst forms and also mention about the irresponsibility of this DMP. Clearly explain that the delay is becuase of that old Dr and request CHC to consider your case and request to resend medical forms. CHC will understand...at the same time lodge complaint against this Dr...
