I'm confused about this, mainly the additional family information form. (5406).

I'm divorced and I don't know how to fill in this form. If i don't fill anything at spouse, it sends me to a note where I must certify that i don't have a spouse or ex-spouse. But i do have an ex-spouse (he's not dead Smiley). Do I have to fill him in the spouse section and mention we're divorced?

Please help.

Just fill in the section where it asks you whether you were formally married

This one cracks me up, I can't believe CIC still hasn't fixed this!!   Perhaps it's a reading test...

I wrote "see attached" in both places, and wrote an explanation that I couldn't fill in the spouse because I don't have a spouse, but that I couldn't sign the statement either because I have an ex-spouse.  I then gave the info for my ex-spouse, and the date we were divorced, and signed my self-prepared statement. 

As if ...
