please advice me about IMM 5562 form (Supplementary Information: Your Travels) I have to fill it once so this will include all my family or we have to fill it three times so three form for me for my wife and my son whos over 18 years old pls advice me.

BUT U KNOW IF U LOOK AT THE TOP OF THE FORM IT MUST BE FILLED BY PRINCIPAL APPLICANT AND IN THE CHECKLIST IT MUST BE FILLED BY ALL SO ITS REALLY CONFUSING AS WELL AS I DONT LIKE TO SEND EXTRA PAPER AS THEY ADVICED IT WILL DELAY THE PROCESS SO PLZ ADVICE

hi,
no, there is no need of extra paper, the form itself will do unless u have a very lengthy travel history.
the form has 2 pages: in the 1st page, u can fill ur own travel history (a. You), and ur spouse's TH (b. Your Spouse or common partner). And in the 2nd page, u can fill ur son's TH (c.Your dependent child 18 years old or older).

hi hman28, there won't be any problems in your case. we are here talking about those who have submitted their application after 18 July 2011. As you have already sent your Medicals and VO hasn't asked for it (or may be you have already sent your travel history it in another printed papers), you need not worry about it. Good luck with your Passport request and Visa.
