Now i need help in furnishing a cover or a submission letter for the case..what all should i write in the cover letter?? what all other documents do i need to attach for this kind of application?

and what all do i need to fill in the open work permit application??is it the same application as for the work permit??what all do i fill in the information asked?? for ex:where it's required to fill in job details,etc...
