Urgent Help needed - for the PR FSW class VO London (i am from dubai)

Good Day.

Just now i am checking the application form and found that lots of forms are changed after 18 july 2011.
Just want to know that for applying under the Permanent residence - Federal skilled worker class and the visa office - London we have to submit all the latest form to the CIO only as per the check list IMM5612 (07-2011)E.

i have one more confusion. Last time when i browse the website i found checklist for the PR-skilled worker Visa office specific instructions - London checklist. [IMM 7023 E (02-2011)]. but 2day when i am checking i am not getting this checklist.
Please advise who are applying under the PR-FSW class.

We have to send the checklist as per the CIO IMM5612 (07-2011)E or any other checklist we have to include.
Urgent help is needed in this matter.
