Updated forms

Hi All,

I recently received the medical request (which I completed). With the request they want me submit updated forms.

Should I pick new forms or old ones (forms have changed since July 2011)? They have asked me send Schedule 1 and now it is called Schedule A. I want to be on the safe side and think new forms should be better. Any comments?
