Do you have to get the forms notarized. What proof of ID do need to send and shoould that be notarized too? I am really confused with this process. When I call the MPS Police access Office the line is always busy. By the way I live in the USA.Thanks for any help I can get.

No it doesn't have to notarized, you just send the original.  Please read the Appendix A checklist for the office where you are applying.

Send the original what PMM. I read the procedure but it is still not clear. I have the form 3019B but do I need to send proof of ID in notarized form. Thanks for you response.
