Anyone applied for TRV extension?

I have received my work permit extension recently,but I totally forgot about TRV.'
Which form should I fill and where should I send?

CIC website has application kits for TRV  outside Canada , but I could not find TRV extension related forms within Canada.

No TRV extensions. You have to apply for a new one outside Canada.

Job_seeker is correct. No TRV extension and you need to apply that outside in canada like Buffalo. The form is IMM 5257.
