Travel history list which includes the date of entry/exit for all the countries visited and the routing, Countries of transit must also be listed with entry/exit dates in a separate sheet...their is no form...
write date country city entry date exit date transit city if any no.of days stayed

create a table in word file (Heading at the top as "Travel History") and write all these things country, entry date, exit date and purpose of visit.
Take the print out on a plane paper and enclose it along with other docs.
There is no separate form for Travel History.

There is seperate form for travel History. SUPPLEMENTARY INFORMATION: YOUR TRAVELS (IMM 5562)In some VO Guides Form is missing but it is mention in the checklist of every VO. Check the link. Paris VO checklist has got it. It is same for other VO's. Take the print of 4th and 3rd Last page of the guide. yes. Bcoz it is mentioned in the VO checklist. I was too confused when I submitted my docs. But after got confirmation from seniors I sent it. Even if you didnt travel you have to fill it. Write N/A in the required fields. It will suffice. |

Hi Guys,
I have sent my application in May 2010. With N/A in the travel history form as I did not travelled any where when I sent the documents. I later I visited another country in June. Do I have to send them the updated form again ?

It is only required when you will submit your full docs to CHC Singapore. At first stage to CIO there is no need of it. |
