Nice analysis, but you missed out two reasons;
(1) Immigration Officer's attitude
(2) Local Visa Offices' working attitude

these two counts alot.

I noticed that too.  All the causes of delay in the list puts the blame on the applicant, none on the other side.  Maybe they authored this list.  That's why when I pray, I not only pray for my application to be processed but I also pray for the visa officers and staff who would touch, handle or read my file.  Don't forget to pray for the postman as well so your mail won't get lost along the way.
