Mas maganda po kung may dala na kayong medical certificate/clinical abstract pati po yung copy ng recent test/xray showing na ok na sya para po isubmit doon. If you don't have any, they will likely ask you to submit those via courier. Question: My son who is 7yrs old got a surgery (elbow) at the orthopedic last year (out patient) but he need to make a follow up check-up for the last 6 weeks then undergo therapy for a month but now his elbow is back to normal. My question is do we still need to get a medical certificate on his case, before going to response to his medical request examination by next week. Thank you.

hi guys,
gusto ko lang malaman kung meron ba sa inyo na nag aaply ng temporary working visa and already took the medical exam in Nationwide? i just took my med exam last may 17... in my part yung passport ko ay nasa canadian embassy na kasabay ng pagpasa ko ng application for working visa kasi required... matagal din kaya ang pag process or gaano kaya katagal... thanks....

im new here, i've learn this page by internet browsing...i hope somebody can help me..in fact we will do medical as soon as i receive the papers coming from my husband...in fact im with my daughter (6 yrs old)..my question is it necessary to bring the vaccination card of my daughter sa pagpunta nmin sa National Health System Inc.? kc po nawawala na ung vaccination card ng anak ko dhil ngpagawa ng bahay ung parents ko namisplace na daw..but im pretty sure na she has all the required vaccination...pls help me...

oo kailangan niya yun kahit certification lang sa dr niya napaka impt. niya same here yung sa akin nawala nadin punta lang ako sa dr. ng anak ko then humingi ako certification. i attached kc yun sa medical ninyo, then magagamit mo din yun pag dumating na yung visa ninyo may form dun yung mga listing ng vaccination ng kid ko so kailangan mo pa certified yun sa dr. na nagbigay sa kanya ng vaccination para sa skul requirement. |

anybody can help me where i can download the forms from Nationwide Health System Inc., gusto ko na sana kc madownload to save time for filling up..since i read here in the forum na maraming dapat fill-upan...nakakita namn ako ng site for NHSI kaso ang downloadble lng ung medical forms for Australia...same lng kaya ung sa forms for Australia and Canada
