Nagpa meds kami last Dec 1 sa Nationwide. Eto kwento ko nangyari.

To save time:
1. Magdala ng madaming ballpen
2. Dalhin lahat ng medical certs kung applicable, previous xray kung me previous case ng PTB or exposure sa PTB
3. Kung nagkaperiod ka ( female ) make sure na pumunta ka after 7 days ng iyong last day
4. Mag pa picture na sa labas, 4x passport size lang kelangan, meron naman sa 4th floor kaso hintayin mo pa.
5. Hawakan ang inyong passport at yung ibibigay na checklist sa step 3 at all time

Eto step-by-step

1. Punta sa counter kuha ng forms
2. Fill up ng sangkatutak na forms at ipaste ang mga pictures, i sign na rin ang pictures
3. Punta sa table 2 para ma assess ang forms, bibigyan ka ng ID at checklist
4. Hintayin tawagin para magbayad
 P4,000 15-above
 P2,000 11-14yrs old
 P1,750 11 below
5. Eye checkup, weight, height
6. Interview ng doc, undress para ma checkup ( CPE )
7. Xray
8. Urine at Blood
9. Tatawagin ka sa table 3 kung ok na tapos uwi ka na
10. Bigay ang checklist at id sa lobby

Sana makatulong.

Medyo mahirap pala hanapin ang building kasi nakatakip ang bldg number. Hanapin nyo na lang slimmers world, tabi lang non.

as a point of reference, i had my medical july 26. until now still waiting for our passports to be requested. there are others who have been waiting for 6 months already.

hi i just wanted to asked some questions abt the medical and the application...im done w/ the medical at nationwide last oct.28,2010 and they said that theyve sent my med results to the embassy last nov.18,2010..my questions are..whats the timeline before you get a reply from the embassy requesting abt the next step?and is there a possibilty that my application will be denied?...thank you so much for any response....

hello po  for MR kami with my 2 kids ask ko lang kung kailangan talaga ng record ng vaccines? naiwala ko na kasi parehas, then second question i was diagnosed with toxic goiter 10 yrs ago the nag undergo ako ng radio active last march lng  kailangan ko pa bang i declare  yun? if ever di ba big problem yun? kasi ok naman ako tnx

in my case, kinuha nung doctor yung photocopy ng immunization record ng daughter ko. para safe na din and sure, bring a doctor's certificate for your toxic goiter stating when it was diagnosed, treated and you are cured na. Kung mayron, show proof nung treatment and any tests that showed na wala ka ng sakit.

thank you sa reply ruby! so kailangan nga talaga ng vaccines ng mga kids, regarding sa goiter ko kung di ko nalang kaya i declare di ba magiging kumplikado yun? tnx

hirap kasi kung di magdedeclare ng medical condition, may chance kasi na they might detect it. 100% sure ka ba na hindi na madedetect ng kahit anong lab test ang goiter mo?

for me lang ha, i would disclose any medical condition especially kung nagamot na naman. CIC requires full disclosure kasi.
