Medical examination form and ME instructions are missing

Hello ,
I have applied for PR on November 28, 2016 and acknowledging of receiving my application was on February 22, 2017 ,medical examination request has been sent to me on March 13, 2017 and deadline to submit my medical examination results on April 13, 2017 but the problem that i didn't receive medical examination form neither medical examination instructions and i can't do ME without this form , and my representative in Canada ( as i am applying from outside of Canada ) has sent many emails to IRCC asking them to release forms but unfortunately with no answer from IRCC , if you have experiences such situation like that please share me your case.

If you had the form but not the email instruction, you could go ahead and take the medical exam first. Your case is different and you have no choice but to wait for the form as the panel will need the IME number in that form. Your representative should find a way to contact them. If email doesn't work, call them. Please make sure your representative is an authorized one (regulated consultant) to avoid possible delay due to this reason.
