wow i didn't learn such a thing like that not unless may sakit po sila. no passport request yet kasi we just comply the additional requirements. how many months po after they ask for ppr?

When they send the kit to LC2, it means there is already initial approval on LC1, meaning, they are qualified to sponsor spouse/dependents. In many cases, the LC1 Open work permit is issued around this time.
It seems your first medical was asked too soon, unless you submitted complete/satisfactory documents the first time. Complete means, you gave all what they asked for. This does not prevent them from asking other documents in the future, though. I guess you were given the AOR (acknowledgment of receipt) at this time, which signals the start of processing (whether shown on e-cas or not). The probability of PPR within the next 2 months from medical is very common. Maybe you missed on some documents specially the cenomar. Did you indicate all purposes as "for visa canada"?
They asked for additional docs in oct 2011... this is rather too late to ask for additional documents since you had your medical almost a year earlier. Normally, they would just ask for updating. Anyway, the new medical and NBI means PPR is coming anytime soon, maybe within 2 months. Take note of this: The passports when submitted, should have your NBI issued not more than 3 months earlier, and the medical with remaining validity of at least 4 months.
When the PPR is released, and the passports submitted, the "real" time count begins. It may be from 4 to 6 months for the visa; unless you are in for another round of delays, which again puts a risk on the validity dates of nbi and medicals. I hope this does not happen. It is already somewhat cruelty to make this happen. Don't worry about the 5th medical....only the living dead goes through this.
From the time the passport/s are submitted, only then can you say that LC2 documents are complete and screening/investigations already done successfully. Finalization process shifts to LC1 at the CIC. This will involve a landing interview and payment of RPRF (pr fee for all of you). Written notice of the interview will be sent to LC1 in many cases 1 month in advance. Your LC1 becomes permanent resident during this interview. After this, the passports with visa will be returned to you from 1 week to within 1 month.
Read/understand very well what the letter of interview to LC1 says. It could be for the purpose of clarifying/verifying information regarding LC1/LC2 documents, not landing interview. There could be discrepancies as LC1 is handled by CIC, and LC2 by the foreign embassy (CEM in our case)
