My spousal application has been submit to the visa office in buffalo. Received a letter from the visa ofice for additional documents such as my I-94 US visa and a letter from guarantor to prove that i enter US legal and stayed for a year Which i provide. Now i received letter from the visa office say that those document are not acceptable so they transfer my file to New Delhi. I would like to know if i have to go back to india, whether i have to do the medical exam again, do i need to contact the office in New delhi for change of mailing address. please help.

First of all, no, you don't have to redo the medical just because your file got transferred to ND, and you don't have to go back to India just yet (unless they request an interview). If your address is still the same as what you put on your application, you don't have to worry about an address change either.
All this means is that Buffalo determined that you did not meet the requirements for having your application processed through their office (
initial entry
to the USA had to be for
at
least
one
year
), so they have sent the application to the visa office in your home country.
The one main issue this creates is that, apparently, it is not legal to mail your passport to India from the US or Canada - so when they request your passport to issue you the Confirmation of Permanent Residence near the end of the process, you're going to have to take it to India. In the meantime, you can stay where you are. New Delhi is actually processing aps faster than Buffalo - so it might not be all bad that it went to ND. They're finalizing in 2-6 months where Buffalo is finalizing in 3-9 months.
