I got the letter of refusal for my application for TFW visa.  My question is, I want to apply now as FSW immigrant as I am eligible to.  Will they require me again to have the medical exam if I applied before the 12th month validity of the first medical that I had?

Yes, the medical is only valid for a year, and must be valid at the time of landing.
