It's the same form used for GCMS notes, but you'd obviously check off the third option in Section B of IMM5563.
How do I go about getting the FOSS notes ?
Sounds like a good idea seeing as I'm going to take the risk !!

Well, I'm still here in Canada
My husband and I decided its not worth the risk !
I couldnt bare the thought of being away from my husband for our Sons first christmas, or worse still stuck in a detention centre  Sad

So I guess only the January applicants have AIP at this point? If so, you've got maybe until this summer to wait. Sorry your Christmas couldn't have been more special. I hope your father is doing well.

Plans changed again !!
I fly out on Jan 22nd.
My Dad is going down hill fast. I have only one Dad and think I would regret it if he never got to see his grandson or me.

We know its a huge risk but ......

I have an outland application filled out and ready to send if they do refuse me re-entry. Hopefully I won't have to use it !!
 
We had a good christmas with our little guy.
I'm. Just happy that we got to spend it with hubby.

I will update again. Hopefully from back here in Canada !!

What a hard time for you! Well, at least you got to have Christmas over here.  Just bring as many docs as you can to show you have applied and even something from your Dad's doctor maybe on the return journey?  I'm not sure it will do any good, but at least you can try.
