hello

in medical exam do asked me about my surgical operation history?

if i had not very important surgical operation in hospital (without blood transmission)
can it cause i undergo hepatitis C test?

They will take blood and urine for testing no matter what you say. 

They also do a chest x-ray and ask about any surgery you had that required you to stay overnight in a hospital.

Yes.  This is from the Dedicated Medical Practitioner Handbook..

7. IMM 5419 – SECTION B: FUNCTIONAL INQUIRY GUIDANCE
Question 8: HEPATITIS

When there is a history of hepatitis, the details from the applicant should include the date and type of hepatitis.

Note: If the applicant is NOT excessive demand exempt, then the results of serum AST and ALT should be provided. If these are abnormal, then a specialist's report including hepatitis C antibodies and hepatitis B surface antigen should be provided.
