Members,

Please tell me regarding the supplimentory information (your travels form) there are 2 pages 1st is for principle applicant and spouse and 2nd is for dependant chidlren over 18 years of age which is not applicable for me, actually I attached only 1st page and already sent the courier I didnt put 2nd page so I am just thinking because of not attaching 2nd page my application might be returned.

if do you think its a serious issue then I can try to locate the courier I have just sent it today..

Please suggest..

Well apparently no harm as u have already mentioned that u do not have kids more than 18 years old. But if u can track down / locate courier and add this page, would be beneficial. Another way out is send a clarification email / post to CHC with reason for skipping the page and attach the 2nd page with the letter. Purpose served
