Study Permit Questions(Help)-International Student Currently Residing in Texas

Can anyone help me with the following questions:

1.Which location do I send my application to?

2.Which location is faster or better at processing study permits?

3. Will I need a Police clearance?(Originally from Zambia, lived in Poland for two years as a medical student before I cam to study in the US)

4.Will I need to take English or TOEFL tests or is this just a Permanent Residence requirement?

5. One the requirements it says they need an original passport, and original i-94 card, which is not a problem for me to provide, but it does not specify whether the i-20 must be original or certified/notarized copy. I am told the only thing they send me back is my original passwport. I can't provide my original i-20 if I won't get it back from them. Does anyone know what I need to do here?
