Sydney, are u referring to the Alberta H1B stream category?
If YES, then you don't need to submit IELTS.

Does anybody know why some letters of invitation say it may take 60-90 days while others say it may take 40-60 days to process?

40-60 days letters are the first stage, as soon the demand increase they start to send letters 60-90 days to avoid backlog, but I think they already have a backlog, they didn't spec so much demand on this H1B program.

Long back in this forum a related issue was explained very clearly.
However, you do not need visa stamping. You need your I797 and a letter from the employer saying you are really working there.  And of course your pay stub W2 etc.
good luck

Additionally... I94 is attached to I797 noticed approval... That is what I sent because I do not have current visa stamped.
BTW I did include my previous H1B visa just in case...

I received letter on October 17 2008.

Please Note that the invitation letter clearly mention following:

We ask that you allow approximately 60-90 days processing time. It is important to note that processing times are heavily dependant upon the volume of applications received and can therefore vary. Please do not call to inquire about the status of your application as this will increase our processing times. Once a decision is reached you will be advised.
