did you also apply as common-law inland?
have you contacted Immigration office and asked what cause the delay?
for my case, there is nothing wrong with it
i sent all the necessary requirements and my file is not even transfered to my local office
it's just being delayed for no reason

"We sent you medical instructions. To avoid delays, please provide us the information requested in the letter as soon as possible."

Do i have to do medical check again?

If it has been more than one year since your medical was done, they will want another medical done before they will issue a PR visa. So, yes, more waiting....
