Hello there,

I would appreciate your help! Urgent!

I am confused with the section 10 of the checklist
for the Permanent Residence Program (Skilled Worker).

1) Where can I find the notarized statutory declaration stating
my intention to proceed to Canada without my family members?
Any particular format?

2) Do I need to fill out the above declaration because I don't have
my own family (husband or children)?

3) I have just had my mother (69 years old)? Do I need to fill out a declaration for her?
   Or confirm anything for her?

If you are unmarried, then this is not meant for you...you dont have to produce any declaration in your case.

However, needs to fill the relevant information about your family members in FAMILY Declaration Form.

That is meant only for married people.
