Follow the link in the first post. At the bottom of the web page there is a link to request the cards online. Enter your address and they will mail the cards to you.

Out of State applicants could request for ISP6-404B form(PCC with fingerprint) or ISP6-405B form(PCC without Fingerprint) either by calling 815-740-5160 or going to

They will send a barcoded form to your mailing address.

Why can't we request PCC without Fingerprint? Why should we pay $4 more ($20) for this option and also go to a local police station to have fingerprints taken ($10 in my case) when we can get non fingerprint PCC for total $16? The CIC website does not mention anything about requiring fingerprints.

Can we all go with the cheaper and simpler option whenever it is available? If you have a lot of money and don't mind extra work (going to local police etc), that's fine, but I do not want the CIC to get another idea of making all us have fingerprints taken for State Police Certificate too. They have already given us enough headaches by adding this new rule of requiring State PCC. Thanks.

I wonder what is the point of getting state PCCs while FBI would suffix all that!!! Would the state PCC show something different from the FBI's report or is it just to make us send the appln to CIC a little slower?

i, for one, am going with the cheapest option available. if cic needed fingerprint from every state they would have specified likewise. ;)

I am in the process of filling out the  FBI fingerprint form and have a question regarding the same. There is cover letter also which has to be  filled out with the fingerprint form. The Cover letter is in the PDF format which has to be printed and the details are supposed to be hand written. Here is how the some content in the cover letter looks like..

" I , ______________________, am requesting a criminal history background check for personal review pursuant to 26CFRS16.30-16.34. Please email the results to check to the following address : "

My question here is what does 26CFRS16.30-16.34 mean in the cover letter ?

Let me make some corrections for you. It is 28 CFR 16.30-16.34 NOT 26CFRS16.30-16.34. And please mail the result, not email. They don't e-mail you. For the rest, visit the link below and read through 16.30 to 16.34.

I am currently in NC and if i go for option of PCC with finger prints then do i have to go to NC local police station for finger printing and then send it to IL OR I have to go personally to IL police station for finger printing

Got the form ISP6-404B. Heading to local police station to get fingerprinted. Out of state applicants could get fingerprinted anywhere, no need to go to IL.

Thanx for ur response. So here what I understood about the cover letter for FBI finger printing.We don't have to worry about 28 CFR 16.30-16.34 in the cover letter. We just have to write our name and address in the cover letter  and send it with the credit card details and FBI form . Correct me if I am wrong.
