I mailed them on 6th march and reached them on 9th march. Not yet received letter.

sent by first class mail, not tracking or registered since it's a p.o. box and these things only add to the timeline.

Do we need to send only the letter we get from  "Dept of State Police" or both the letter and form we sent?

since i opted for el cheapo version, i only received the original form with their ok stamp on it.
