As right now i am in michigan i directly got the MI state finger print forms. Please call the number mentioned in CIC website, instead of guessing ourselves. it is going to be for primay and seconday applicant.

Currently , i  am in North carolina , went to local county police department and got my finger prints on FBI CARDS and not on state cards ( i am not sure if you are refering to MI state card or NC state card )

is that ok ?

No cover letter required. Go to local police statation and request for state finger print forms. fill the form and send it to

Include $30 DD payable to "The State of Michigan"
They will send it in two weeks

I got my fingerprints on FBI CARDS for MI PCC but where we are sending it to with $30 ... do we have to fillout cover letter or any forms
whats the address is it the same on the cic website for Mi or somethnig different .....

anybody ?

I called MI police and found following

For out of state only

Go to your local police department and get your finger prints on a card ( FBI or state card )
send $30 cashiers check which includes mailing back fees
no forms or cover letter is needed
Write clearly on the card  - purpose of finger printing ( i.e. canadian immigation )
Leadtime - 2-3 weeks
you can combine multiple application in one envelop and also can make one  draft / cashiers check for all the applicants

i have not applied for it yet .... due to the length of time Alberta is taking nowadays for sending nominations

As of 12/13/2010, the correct process for Michigan (at least according to my conversation with the MI police records division at 517-241-0606) is:

Make check or money order payable to "State of Michigan"

Send check, along with fingerprint card, to
  Michigan State Police
  PO Box 30634
  Lansing, Michigan 48909

Processing time: 2-4 wks
Fingerprints not need be on an FBI card, specifically (but a live scan (digital) is insufficient)
No cover letter is required
One one copy of the prints are required

(I mention that last fact because my local fingerprinting service told me that the FBI clearance requires multiple sets; I'm unsure if that's just their money-making claim, or an actual DOJ requirement (not that I've seen it mentioned on the FBI website))
