Not yet, but I did call them up today. She said I need to send a $24 check and the criminal history information request form. I'm also going to send them fingerprints and the FBI Cover Letter just to be safe, and also mention somewhere it has to be CERTIFIED. (Not sure where to put that yet, though, but I have a few hours to figure this out!)

Yes.. I got police certificate in one week and I sent all the information along with social security number and $24 dollars personal check. I did not send fingerprints.
