Hi,

My wife got her medical form, with her Lastname as her maiden name.
However, she is already using my lastname but her passport is still with her maiden name, there's already an ammended into it.

But when CIC sent her medical form, it has her maiden name on it.
The thing is we already have sched the medical soon, bcoz she's going for a vacation outside the country, and I want to finish the Medical before she leave.

Is it fine to have her maiden name on the medical form and do the medicals and just change the lastname later ?

Is there anything to worry in this kind of scenario ?

No,we had almost the same problem,but our counselor said,we should do the name change after we send back the medicals.Otherwise it would take so long to get new corrected medical forms.You can do it after sending back the medicals.There should be no problem with it.

CIC is basing the name that on the documents, ie the passport.   They don't know that your spouses intention is to use your last name. Get the passport amended.
