Hi,
Yesterday I have received my request for medicals alongwith the medical forms from London Office.
I have noticed that the medical form of my wife contains her given and family name incorrect.My wife's given name  in passport is "Fatima Naeem" and her family name is written as "Butt".Whereas the forms contain her given name as only "Fatima" and "Naeem" as her family name whereas the real family name"Butt" is missing.
Plz advise is this a minor difference and I should request DMP to correct it or should write to London office regarding this mistake.

Dear...u should send a letter to CHC for your observation on error you have noticed through your DMP (part and parcel of Medical reports to be sent by DMP to CHC). Apart from that pls let me know what is written on the passport. FYI only, visa will be issued as per the name on passports and not as per Medical forms but off course common data shall be used during the whole process. But foremost thing is CLARIFICATION / OBSERVATION LETTER TO CHC ALONG WITH MEDICAL REPORTS.

I suggest you to return the medical request and forms to CHC London for correction of her name and to re-issue medical request. That will also correct the entries in their system. Make a covering letter to explain as what you need. Send it by speed post or courier.

Plz advise is this a minor difference and I should request DMP to correct it or should write to London office regarding this mistake.I have been advised by one of my friend to contact CHC about this issue but actually I already received my medicals quite late and  London will take atleast 5-7 weeks to issue another medical form.Moreover I am leaving for vacation in first week of November for almost 40 days so I am thinking it to mention to my DMP and request him to correct it and sign it and send my passport copy to the CHC alongwith the results.Moreover I ll also mention this issue in my cover letter to London after doing my medicals.What is your suggestion in this regard.
Any advice ll be highly appreciated.

Congrats for the Meds!

Note that the space in the Surname Field of all CIC documents has place for ONLY ONE word. So, if one's surname is "Shaikh Mohammed" - it'll be written as SHAIKH only. Unless u ask them to write it as MOHAMMED, while obliterating the word "Shaikh". In any case - it has to be One Word.  

The ideal situation is to club the first 2 words of the name as "Given Name"; and use the last word as "Surname".

Your query's Answer: U don't require to send the forms back. Just ask your DMP to amend the name/s as "FATHIMA NAEEM" -as her Given Name, & place "BUTT" in her Surname field. The DMP can do it (Xchecking your pps), sign & place his seal against the changes. There's a CIC guideline to that effect.

I would suggest that you email your CIC office and advise them of the error. i would also courier a letter to them as well and enclose a photocopy of the form and highlight/circle the error. In my case the photos on my med forms were interchanged with that of my son and my marital status was listed as single. I emailed my CIC office and the reply from CIC when i eventually got a reply was that they have corrected my status on their system. they resent me the correct forms. Hence i would suggest fixing it directly at CIC level rather than hoping that the DMP correction would also fix it at CIC level.
