nga pla, i was asked to accomplish din ung APPENDIX A,(according dun sa sulat nka-attached n un sa letter), just want to make sure na ok na lahat, meron krin b nun when you received the letter from embassy? family members mo tlga pinasulat mo?

bali yung representative ko nagsulat dun nagsign lang ako..di na yung family ko kase ung asawa ko ang sponsor ko tapos ung anak namin canadian citizen na kaya ako lang nagsign

Thank you nice2010. How long do I have to wait para sa CAIPS na mag-respond? Baka kasi too late narin kung mauuna pang ma-expire yung medical nya before sila magrespond sakin...

      sorry delayed akong nag reply hope nandiyan kapa .it takes a month bago ma receive mo yun pero its still useful who knows kung may request pa sila. but im praying hope you receive yung visa nang bf mo this month

 before kc sis when i got my req.for others doc. mga around feb. kumuha ako ng GC sa iabang bnsa kc kailanagn ung pinakita ko lang ung Gc galing don mismo sa ibang bnsa. d ko xpct na e accept nila un...pmunta ako sa office at ng writting letter oh k don..ask ko kung pwd..kc they ask me to file here from in ph tlga soo un...kc dapat 2 months pa kc ko ung makukuha ung gc request ko from other embassy...
 Around april 13 ata ngcol sking ung embassy nasna na daw iba kung papers sbi ko nganaty pa ako ng GC sbi nyah andon an daw..so kinabuksan pinadala ko na agad  .....xnxa na sis...hndi rin ako msyado ngack online kc kakastress  june 3 napnsin ko lng they have my medical result...thanks to good....kakastressful isipin....

Two parts ito:

The applicant in the Philipines fills this up:
cic.gc.ca/english/pdf/kits/forms/IMM5475E.pdf

Then, the person in Canada applies for the CAIPS note using this form:
cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf

Then mail both forms with $5 check paid to the Receiver General of Canada to one of these:
cic.gc.ca/english/department/atip/requests-address.asp

Hi Caips_note, I have a few questions regarding the CAIPS form.. Ano nilagay mo sa Requester's reference number? In my case, do I need to tick the 'Consent included' box? Do I need to tick both of the 'Access to Information Act' and 'Privacy Act boxes'?

Sa section B, isa lang iti-tick ko? Just the CAIPS? Nothing else? Sa required information, ano nilagay mo? Kasi, wala pang visa file number yung bf ko eh.

Sorry ang daming tanong...... Nade-depressed na kasi kami ng bf ko, araw araw nalang kami naghihintay.... I want this waiting game to be over, or at least magka-idea naman kami kung ano na nangyayari sa application ng bf ko.. THANK YOU SO MUCH in advance.
