I also used it as a proof of our relationship, we went to college together so I attached a copy of our transcript of records then highlighted the subjects that we were both in, makikita nila na halos lahat ng subject pareho kami ilan ilang lang ang hindi so I guess they would consider it a proof. =)

Here are the proofs we provided... thought this might help you..  

1. Proof of joint bank account - photocopy of your bank book or statement of account.
2. House Bill -  Electrical bill, Internet Bill, Cellphone Bill, Laundry bill.. Any bills that will show proof of the same address.
This will further prove your are leaving together.  
3. Loan applications /receipts - showing your address
4. Merchandise receipts from botiques, kiosks, and malls... - Everytime we buy something, we ask to put our names in the receipts.
5. Photographs - He have lots of photographs together so I created a timeline / journal from the start of relationship to present.
Each photos with dates/year and short explanation when it was taken.  I think we have a total of  15 to 20 pages of bond paper in our journal.
6. Joint owner of properties - Land, car, dogs/pets.
7. Birthday or Wedding card invitations address to both of you.
8. Letters from your family and close friends duly notarized that will vouch your relationship. That they have known you and
witness your interdependency.
9. Internet communications - gmail, ym, skype,
10. Birthday, Anniversary, Valentine, Christmas Cards that you gave to each other.

Just new here. I have a question regarding dun sa Generic Application form, medyo nalilito lang dun sa field for the personal details under the given name(s) dito ba sa field na 'to kailangan i-indicate yung middle name? Kse di ba nakalagay as exactly shown on the passport.thanks in advance.

I'm new here! may problem kse un husband ko sinabmit un application namen ng daughter ko under spousal class without our medical exam. when i checked my ecas nka sulat na "they started processing our application on march22,2012. ang worry ko lang kelan kaya sila mag ssubmit ng medical request? consider pa ba 9months process kahit kulang ng medical un application namen ng daughter ko? thanks! sana someone can help me.

kng mern ba ung mga card ng asawa mo pwede nya pero i doubt it na ggmtin nya un like now my husband has an iphone so sa viber kmi naguusap dba cheaper pero we cannot send proof:(  kng an mern kay proof pwede na un.. letter .. email !! any will do ..

hmm for i read sa mga messages is naghhntay mga tao ng request for medical .. pero personal kng ano nasa situation mo magpprint ako ng form sa website and magppamedical ako !! ayoko magwait ng request maddelay ung papers ko... kc ssend  nmn ng dr ung proof mo eh then magbbgay sau ng copy na dpat issma mo sa docs mo..  pero of mern ka doubt call mo immigration for help.. ung advice ko sau is ung ggwin ko.. kc what happen to me too is dko nabyran ung permanent fee. so binyran ko na xa and pinadala ko sa mississuaga ung proof of payment dna ko naghntay ng request until now wla pa nmn cla ask ng request and pinadala na ng asawa ko ung passport nya
