Hello,

I am trying to sponsor my british husband who is living in Canada with me. We have gotten back his police check and are putting together other required information. A few questions:
1) will he need to do a medical exam with the out of country process, or only if requested?
2) I am in the process of changing my last name (done so far on driver's licence and a few other documents) but a lot of my documents are in my maiden name, what name should I apply in? is this going to cause an issue?
3) when looking at information on sponsoring in family class through buffalo, i believe at one point i read we'd have to send forms to mississauga instead, is this right?
4) i believe if processing through buffalo we would submit payment at the time of application, is this right?

Everyone has to do a medical, you have to submit the receipt portion of the IMM 1017 the DMP gives you with the application.

Not if you submit the marriage license which shows the name change.

3) when looking at information on sponsoring in family class through buffalo, i believe at one point i read we'd have to send forms to mississauga instead, is this right?

Please read the instructions thoroughly, you send the sponsorship and the completed application with all the attachments, police certificates, proof of relationship medical exam, to Mississauga.  then if it s approved, Mississauga will send it to Buffalo.

4) i believe if processing through buffalo we would submit payment at the time of application, is this right?

The Government doesn't work for free, you have to pay the $550 fee upfront, though you can delay the $490 right of permanent resident fee until the passport request.

I took this to mean that we could submit the application directly to Buffalo? Is this not the case?

No, it means that if you are admitted to the US or Canada, your application can be processed at Buffalo, it still have to go through Mississauga.

One other question. I have now paid the fees (including right of permanent residence) online to the Missisauga office. The official receipt we need to fill out. I made the payment (I am the sponsor) so do I put my details on the receipt or should it be the person I'm sponsoring? Also, what would I use as a client ID. I have not submitted anything yet, so I do not have this. Would I leave it blank?

Also is this official receipt I printed after making the payment the IM 5401 form or do I need to order that separately?

If you paid online you would have received a pdf file in your email.. this is the receipt you attach to your application. You don't order any other receipts from CIC.
