Spelling Mistakes In medical Form

There is a minor spelling mistake in my daughter name in the medical report form  . It is written Yashika instead of Yashica

In the letter, the authorities have stated that we should notify them if there are significant errors. I m not sure about the intensity of error which makes it "significant"...

Please advice me ASAP

Hi,

I had a similar case with my daughter's medical. I contacted the London office and they advised me to proceed with the same form and the mistake would be amended on their system. So nothing to worry about really, but I suggest you send them a mail before proceeding.
