 Hi!
I would like to know whether the B4, SIN Card and OHIP application forms should be filled and then printed or vice versa?
I want to prepare them beforehand in order to save time.

You can do either, but they prefer forms with printed information.

I guess that your writing is neater than mine Smiley

They do accept handwritten and electronically filled forms, just make sure that the ink is blue or black.
I know a couple of people who used green (despite what the form states) they were handed them back (rejected).

What about printing? Roll Eyes  The applications should be printed two-sided or on separate sheets?
