Has anyone being asked for "Signed and Sworn declaration" in a an affidavit form? Is this suppose to be notorized or something that one would create and sign?

An affidavit is an official form that someone has designed, so you need to locate it, complete it, and sign it in front of a notary public or somone else appointed to witness signatures.

The only affidavit I've had to sign was the declaration of eligibility to mary, which had to be signed in  front of a Canadian consul.  If this is your case, make sure you sign in front of the correct consul. In China, each  consulate serves specific areas of China, and China won't accept an affidavit witnessed by the "wrong" consulate, even if it is in China.

Isn't bureaucracy wonderful!!??

But it says create in an affidavit form not an actual affidavit.  They never send any form to anyone but asked applicant to make one.  This is from CHC-ND

I am not familar with the abreviations.  WHat is "CHC-ND"?

The requirement says....

Signed and sworn declaration in an affidavit form, identifying all of the principal applicant's children (natural and adoptive) and brothers or sister (natural and adoptive), giving full name, date of birth, place of residence, marital status, years of education successfully completed, and the name and locality of schools they have attended.

It sounds like they want a normal declaration, in whatever form you wish.  But wait for more experienced people to answer, please. I don't want to mislead you.

This for sponsorship application after the appeal is allowed . New delhi send this request but they never asked anthying to attest. Here is there verbage...  Please advice...

1. Signed and sworn declaration in an affidavit form, identifying all of the principal applicantīs children (natural and adoptive) and brothers ans siters (natural and adoptive), giving full name, date of birht, place of residence, marital status, years of education successfully completed, and the name and locality of scholls they have attended. This affidavit must be provided even if an affidavit was previously submitted.

The affidavit must also incorporate the following paragraph:
"I further solemnly affirm and declare that the particulars of all my sons and daughters (natural and adoptive) and brother and sisters (natural and adoptive) have been correctly stated and that i have no other son or daugher (natural and adoptive)/brother or sister (natural and adoptive) in India or abroad, from any of my marriages/any of the marriages of my parents".

I went to a lawyer to get my affidavit. His secretary asked me what the affidavit had to say, then typed it up in the correct legal form. I then saw the lawyer, who asked me if I swore that everything in the affidavit was true to the best of my knowledge. I said yes, then we both signed it and he stamped it.
