    You don't need any forms. To avoid any errors in providing the address on phone, you can first change the address online at https://services3.cic.gc.ca/ecas/?app=coa&lang=en and then call them to confirm the same.

    You can land at any POE. At the most they may ask you all sort of Q's. If you can get Alberta address that would be good but if that's not an option then it should be a problem As long as you have valid I797 AND 94, using AVR shouldn't be a problem.
