yes you are right...I had read that things on Qorax forum...but when i asked him personally .. He told that the information i read was out dated and nowadays there is no such need to fill up any forms or anything as such :) thanks for your concern..

brother i am not sure about your pharmacy council but in Gujarat its mandatory to have visa over the passport for the good standing certificate... so before applying for document evaluation check it out with your pharmacy council..!! Hope this helps.. :)
