Schedule 3 FSW..... One box left blank

Hi Seniors.


Refer to Schedule 3 Economic clasess-FSW.

When we fill up 1) Your Full Name there are three Boxes.

Family name    {BOX 1}

Given Name(s)  { BOX 1}
                     {BOX 2}

                   
One Box is for Family Name wheras Two boxes are for Given Name(s).

I wrote my Family Name "Hussain" in one box while "Manzoor" as Given Name in one of two boxes for given name(s). There left one box blank.

What to write in this box which left blank?
