Requirements for Passport for Visa Stamping (FSW1).

Hello everyone,

I received Passport request on January 16. I have a few questions before I send the documents.

When sending passport for visa stamping:

1. Regarding the photographs, for the visa (NOT initial application), are these the only requirements:
Size 45x35 mm, head centered, head 31-36mm, top of shoulders showing, neutral expression (no smile), good lighting etc. I just want to make sure there is NO requirement to write/stamp the date/name/DOB etc on back of photograph.

2. I took a money order from my bank. The payee, payor, and amount are clearly noted. Is there anything I am missing?

3. For the height/weight/eye color information that is requested for stamping. Is there a specific form to fill out that information? Can we just print out the email that was sent and enter the information?
