Why cannot vo reject applicant if they catch fake documents or false employer reference/ security issue in applicants background checks?

Yes, they can. If documents submitted were proven or suspected to be fraudulent or fake. Or if the VO assessed your file/qualifications and felt that you did not perform the required duties and responsibilities/lacked minimum points. background checks are started soon after your VO gets your full docs.
Once you reach IP stage, you are set to get MR or get the Rejection.
IP is the stage when your VO could make calls to your previous employers or ask for further documents.
